Do We Accept Medicare?

Yes, we are a Medicare provider. Medicare will cover 80%. The remaining 20% is the patient's responsibility, unless you have current secondary/supplemental insurance. As a courtesy to you, we will also bill your secondary/supplemental insurance.

Are We a Medicaid Provider?

No, we are not.

Do We Accept Private Insurance?

We do accept most health insurance plans. Being a participating provider does not guarantee payment for services, and patients will be responsible for charges applied to a deductible, co-insurance, co-pay or any non-covered charges.

Do I Need a Referral/Prescription From My Doctor?

All MEDICARE patients require a referral from their physician. Each referral/prescription is valid for 30 days from the date of prescription.

Not all PRIVATE INSURANCE companies require a prescription. It is important to remember that all health plans are different and to ensure that you receive coverage according to your plan, you need to call your insurance company to verify. Our staff will also call to verify benefit coverage for you as a courtesy but we are not responsible if they provide us with incorrect information.

Florida is a state with DIRECT ACCESS, meaning you can be evaluated and treated by a licensed Physical Therapist for up to 21 days without a physician's referral/prescription. However, MEDICARE does not cover direct access PT, and some PRIVATE INSURANCE payors may also require a referral/prescription.

How Do I Check if My Therapist is Properly Licensed in the State of Florida?

Go to the website, My Florida.com and search for the therapist by first/last name and profession.

What do I need to bring with me on the first visit?

For the initial evaluation, you will need to bring your insurance card, drivers license, and list of medications, if applicable. You may download your patient intake forms from our home page and bring your completed forms with you at your initial visit. Loose, comfortable clothing is best for the evaluation.
